Student Academic Reference Form ​
The reference should be completed by a member of staff at the student's current or most recent school, preferably by the Head of Year. We ask the school to return the completed reference form by email to admissions@sfx.ac.uk, or to post it to Admissions, SFX 6th Form College, 10 Malwood road, London, SW12 8EN.  Any forms sent by post or being handed in by the student must be stamped with the school stamp.
